Application Forms

• Sidewalk Cafe Permit (PDF) (Chapter 6.47 Grants Pass Municipal Code) Fill out the form, print and sign the form at Community Development Department.
• Sidewalk Cafe Permit Parklet Addendum (PDF) Fill out the form, print and sign it, and bring it to the Community Development Department. Must also have an issued Sidewalk Cafe Permit.
